When enrolling your child in the Liberty Elementary School District, you will need the following items:
A completed enrollment packet - make sure all pages are filled out and that you have signed the enrollment packet. REMEMBER- only a parent or guardian may enroll a student.
A certified copy of your child's birth certificate.
One proof of residency which must be a current utility bill, lease agreement, or notarized letter indicating that you are residing with another family.
A copy of up to date immunizations.
A copy of your child's withdrawal form if you are coming from another school in Arizona.
